目的 通过分析飞行员的前庭功能稳定性的评定等级和影响因素,为前庭功能稳定性训练提供参考依据。方法 随机选取2023年6月至2024年12月在某单位参加前庭功能稳定性训练的213名男性飞行员为研究对象,其中三代歼击机飞行员131人,二代歼击机飞行员82人。根据前庭自主神经反应分级表和前庭功能稳定性评定标准对前庭功能稳定性进行评定,采用多元logistic回归分析法分析年龄、机型和飞行时长对飞行员前庭功能稳定性的影响,同时记录训练前后的血压、心率。结果 213名飞行员的前庭功能稳定性训练评定等级均在合格以上水平,合格率为69.48%,良好率为11.74%,优秀率为18.78%。以合格等级为参照组时,与年长飞行员相比,年轻飞行员获得更多的优秀等级(OR=0.559,P<0.001);与二代歼击机飞行员相比,三代歼击机飞行员获得更多的优秀等级(OR=4.815,P=0.003);飞行时长对优秀等级无影响(OR=1.003,P=0.076)。与训练前相比,训练后飞行员的收缩压和舒张压均明显升高(P收缩压=0.034,P舒张压=0.042),训练前后心率相比较,差异无统计学意义(P=0.955)。结论 参训飞行员前庭功能稳定性等级为合格的比例较大,应注重航空生理训练卫生宣教,提高训练积极性,重点加强年长飞行员和二代歼击机飞行员的前庭功能稳定性训练。
Abstract:Objective To provide data for vestibular stability training by analyzing the grading and influencing factors of pilots' vestibular stability. Methods 213 pilots who participated in vestibular stability training from June 2023 to December 2024 at a certain unit were randomly selected. Participants included 131 pilots of third-generation fighter aircraft and 82 of second-generation fighter aircraft. Vestibular function stability was assessed using the vestibular autonomic response grading scale and the vestibular function stability evaluation criteria. A multivariate logistic regression analysis was used to evaluate the impact of age, aircraft type, and cumulative flight hours on stability outcomes. The changes in blood pressure and heart rate before and after training were also recorded. Results All 213 pilots achieved vestibular function stability training assessment ratings at or above the qualified level. The pass rate was 69.48%, the good rate was 11.74%, and the excellent rate was 18.78%. When using the qualified grade as the reference group, younger pilots achieved more excellent grades compared to older pilots(OR=0.559, P<0.001). Compared with second-generation fighter pilots, third-generation fighter pilots achieved more excellent ratings(OR=4.815, P=0.003). Flight duration had no significant effect on excellent ratings(OR=1.003, P=0.076). Post-training systolic and diastolic blood pressures were significantly higher than pre-training values(P=0.034 and 0.042, respectively), whereas heart rate showed no significant change(P=0.955). Conclusion A substantial percentage of participating pilots achieve only a passing level in vestibular function stability. To improve training outcomes, aviation physiological training education should be reinforced to enhance pilots' motivation, with particular focus on vestibular stability training for senior pilots and second-generation fighter pilots.
